Everything you need to know before your visit.
Booking appointments
Appointments are booked by phone at 416-485-9111. Please let us know the reason for your visit so we can book the right amount of time. If you have several concerns, ask for a longer appointment.
If you can’t make your appointment, please call as early as possible so the time can be offered to another patient.
What to bring
- Your valid Ontario health card (OHIP) — required at every visit
- A list of current medications, vitamins and supplements
- Any forms that need to be completed
- Your questions — writing them down beforehand helps
Prescription renewals
The easiest way to renew a prescription is to ask your pharmacy to fax a request to 416-485-3232. Some medications require a visit before they can be renewed. Please don’t wait until you run out — allow a few business days.
Test results
Dr. Hopkins reviews every result. If a result needs follow-up, the office will contact you. You can also view many Ontario lab results yourself through your lab’s online portal.

- Emergency: call 911 or go to the nearest emergency department.
- Health advice after hours: call Health811 by dialling 8-1-1 (TTY 1-866-797-0007) to speak with a registered nurse, 24/7, or visit health811.ontario.ca.
- Nearby hospital: North York General Hospital, 4001 Leslie Street.
